Online Athletic Clearance

Quick steps for parents/students using the online athletic clearance process.

Visit www.AthleticClearance.com
Register. Parents register with a valid email username and password. You will be asked to type in a code to verify you are human. If this step is skipped your account will not activate.
Login
Select “New Clearance” to start the process.
Choose the School Year in which the student plans to participate. Example: Football in September 2021 would be the 2021-2022 School Year.
Choose Carson High School (If you choose another school we will not receive your paperwork)
Choose Sport
Complete ALL required fields for Student Information, Educational History, Medical History and Signature Forms.
Download, Fill Out, & Upload Private Automobile Driver Certification of Liability Insurance AND Transportation Waiver Forms.
Upload a copy or photo of your child’s Medical Insurance Card.
Once you reach the Confirmation Message you have completed the process.

IMPORTANT

If you would like to register for additional sports/activities you may check off those sports below the Confirmation Message. Electronic signatures will be applied to the additional sports/activities.

Once all requirements have been submitted and uploaded, print the confirmation message page, the parent/guardian must sign it, and turn it in to your head coach. Students will not be processed and cleared unless this signed confirmation message page is turned in.

All of this data will be electronically filed with your school’s athletic department for review. When the student has been cleared for participation, an email notification will be sent.

What is your Username?
What if my child plans on participating in Multiple Sports?

Once you complete a clearance for one sport are arrive at the Confirmation Message, you will have the option to check off additional sports/activities for the current school year. If you decided to participate in an additional sport/activity later on, you can access the multiple sport check boxes by clicking on “Print” under the Confirmation Message of your original Clearance for that specific year.
Physicals

The physical form can be downloaded and printed from Carson High School’s website, www.carsonhs.org and can be found on the Athletics page under the Athletic Clearance & Forms tab. Students can also pick up a copy from the school nurse or Mr. Kline. DO NOT upload a copy of your physical online. Turn in your completed physicals to the drop box located in the main office on campus. We cannot accept emailed physicals.
Why haven’t I been cleared?

Once all required documents are completed, uploaded, and the signed confirmation message page has been turned in to your head coach, then they will turn in the paperwork to Mr. Kline. Then Mr. Kline will review the information you have submitted and either Clear or Deny your student for participation. You will receive an email when your student is cleared.
